Enrollment
After carefully reading the Voice Instruction Information Sheet, complete the Lesson Enrollment Form and return the form along with your first payment in an envelope to your choir director. Space is limited, so please do this immediately.

Lesson Rate and Payment Schedule
Each 30-minute lesson is $17. The payment schedule is included in this packet.  Please select one.  Mrs. Montgomery prefers that students in her studio mail the payment to her address.

Late Payment
The fee for a late payment is equivalent to one voice lesson ($17).

Number of Lessons During the Year
15 weekly lessons will be scheduled during the Fall semester, and 20 weekly lessons will be scheduled during the Spring semester.

Absence From Lessons
Absences happen and things come up.  If you know you are going to be absent, then please contact me at least 24 hours before the absence.  I have provided my e-mail on this form and the payment form.  You may e-mail or call me; the e-mail should be sent before 7 A.M. on the day of the lesson.  An absence on the day of your lesson without prior notification for reasons other than an emergency will not be made-up and no refund will be given. Please use text messages ONLY when the message is very urgent.

Makeup Lessons
Makeup lessons will be scheduled for students whose absences are reported within the required time frame.  We will schedule a time that is convenient for the student, teacher and parent. This applies to both teacher absences and approved student absences. If there is a teacher absence that cannot be rescheduled by the end of the school year, the student will receive a refund for the lesson. If the student has rescheduled a make-up lesson and does not attend it, the teacher is not required to schedule another make-up time or refund any tuition. Up to two student absences can be made up in a semester.

Additional Lessons
These are quite common.  Students who are nearing auditions (all-state, college, muscials), competitions (solo and ensemble, NATS), or specific performances are encouraged to take additional lessons.  These can be scheduled by asking.  Payment for the additional lesson(s) is due on the lesson date.

Materials Needed for Lessons
In general, always bring your music, pencil, and water in a closed container.  If you do not have a piano at home, you should invest in a pitch pipe – these are inexpensive and indispensable; students need to have something to give them their pitch and to keep them on pitch!

Practicing
By enrolling in voice lessons, your choir director and voice teacher assume you want to strive for excellence. Practicing is a part of learning to master any skill. Do your best to be prepared for every lesson to get the most out of your time. If the student is consistently unprepared, their enrollment in the studio will be re-evaluated.

Ending Enrollment
This is strongly discouraged.  When you sign up for lessons, a schedule is created which includes you.  When you drop, the schedule must be reworked.  Lessons may be cancelled at the end of each school semester by notifying your voice teacher with a signed note from a parent/guardian and student.


Voice Lesson Payment Information

Please read and retain this information for your records.   
Below is a choice of payment plans and schedules.
Choose one and specify your choice on the Lesson Enrollment Form.

Checks and cash are both acceptable forms of payment. 
Checks should be made out to your assigned voice teacher (Stege, Priddy or Montgomery)
Your selected payment plan will specify when to submit payment, and how much is owed.

PAYMENT PLANS

Payments are based on the total cost of 15 lessons for the Fall semester and 20 lessons for the
Spring semester for a total of 35 lessons during the entire school year.


Option A     Full Year Tuition Payment
Full year tuition is $595.00 and it is due with the enrollment form before or at the first lesson.

Option B    Semester Tuition Payments
Semester tuition is $255.00 for the Fall semester and $340.00 for the Spring semester. 
Payments are due the first lesson of the semester.

Option C     Bi-Monthly Tuition Payments
Payments for the Fall semester are $127.50 and are due in September and November.
Payments for the Spring semester are $170 and are due in January and March.
Payment is due at the first lesson of these months.

Option D     Monthly Tuition Payments
Payments for the Fall semester are $63.75 and are due in September, October, November, and December. 
Payments for the Spring semester are $68.00 and are due in January, February, March, April and May 
Payment is due at the first lesson of the month.
